Job summary

Northern Case Management is seeking an Associate Clinical Case Manager to join our team in the Bury and North West area. The role focuses on networking cost-effective services for clients, assessing needs, planning care, and advocating for clients in home and community settings.

The successful candidate will hold a UK health qualification and have experience with neurological conditions, with duties including managing care regimes, supervising support workers, and coordinating rehabilitation

Qualifications

  • UK professional health qualification required.
  • Experience with neurological conditions in community settings.
  • Knowledge of MCA/DoLS and safeguarding frameworks.
  • Full UK driving licence.

Responsibilities

  • Provide & maintain a case management service to a range of clients, families & Deputies within all NCM policies and documents.
  • Assess Client Needs and Risks and produce a Case Management Plan with client/representatives prior to intervention.
  • Produce action plans and goals with timeframes specific to the client.
  • Establish and maintain a positive therapeutic relationship with clients, families, Deputies and solicitors.
  • Develop and implement a Support & Rehabilitation Plan with guidelines for support workers.
  • Recruit, select & manage care/support workers.
  • Undertake other duties to meet service needs.
  • Ensure service user needs and business objectives are met.

Job description

Northern Case Management have been established since 1997, founded to support people with acquired neurological damage (i.e. brain & spinal cord injury). In 1997 this level of support was almost unheard of but has since become an integral and incredibly important element of supporting people with acquired neurological damage.

We have an amazing opportunity for an Associate Clinical Case Manager to join us as at Northern Case Management.

Main focus Location: Bury & North West
We are looking for experienced Paediatric and / or Adult Clinical Case Managers.
About the role:

The primary role of the Clinical Case Manager is to network cost effective services and resources on behalf of our clients to bring about an improvement in their quality of life. This includes assessing, facilitating, planning and advocating for their needs on an individual basis. This ensures that management is Client focused; that the most appropriate services are accessed; and that continuity, consistency and structure are maintained. Our work includes setting rehabilitation goals with Clients and support workers in the home environment; facilitating the client's and family's adjustment to the injuries; recruiting, training and supporting support workers; and managing care regimes.

  • Full time, 37.5 hours per week
  • Mon to Fri office hours

What you'll be doing:
Some of the main roles and responsibilities:
  • Provide & maintain a case management service to a range of clients, families & Finance Deputies working within all NCM Policies, Procedures & supporting documentations.
  • Assessment of Client Needs and Risks and Produce Case Management Plan and agree with the client and/or their representatives prior to providing active intervention.
  • Produce action plans and goals with timeframes that are specific and relevant to the client.
  • Establish and maintain a positive therapeutic working relationship with clients, families, Deputies and litigation solicitors.
  • Produce and implement a Support & Rehabilitation Plan with specific guidelines for support workers to implement the support/rehabilitation program.
  • Recruit, select & manage care/support workers.
  • To undertake any other duties appropriate and as reasonably required, to meet the changing needs of the service.
  • Ensuring service user needs and business objectives are met.
What you'll have:
  • Hold a UK professional Health qualification ie, nurse, OT, Physio Social Worker etc.
  • BABICM / IRCM registration desirable
  • Have some experience of neurological conditions and working in a community setting.
  • Thorough knowledge of the CQC fundamental standards and quality statements.
  • Full working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Safeguarding frameworks.
  • Ability to work as part of a team and as an individual. To provide direction support workers.
  • Self-confidence and able to manage challenging situations.
  • Ability to address and respond to employee issues in a fair and consistent manner. Including informal and formal performance management.
  • Excellent interpersonal, verbal and written communication skills.
  • Strong organisational, time management and planning skills, with a commitment to self-management and continuous personal development.
  • IT literate. Use of MS Word, Excel and Outlook.
  • Flexible, professional and positive approach to multi-site working.
  • Proven reliability in terms of full attendance and timekeeping.
  • Full UK driving licence.

Northern Case Management work closely with the client and their family or representative to provide tailored support packages for clients in their own homes and in the community.

NCM has continued to expand to meet the increasing requirement for case management services and now boasts a multi-disciplined team of 10 case managers plus support staff at head office and manages over 250 care/support workers on behalf of Clients. NCM is determined to continually develop in order to provide intervention for as many people as possible.
