Qualifications

• Assistant Practitioner Qualification or equivalent.
• NVQ level 3 or equivalent.
• Evidence of continued professional development.
• Knowledge of long-term conditions.
• Knowledge of mental illness and disability.
• Knowledge of the cultures and religions of the area communities.

Responsibilities

• Support the Occupational Therapists and Physiotherapists within intermediate care, undertaking non-complex assessments and carrying out treatment plans under therapists' direction.
• Work across a range of settings and support OT/PT interventions within intermediate care.
• Travel to patients' homes for visits and work with patients in short-term beds and dementia care units.
