A healthcare provider in Pontypridd is looking for an Assistant Practitioner for their Falls Prevention and Management Therapy service. This is a fantastic opportunity for an assistant practitioner to join the team as we expand our service. You will be a part of the falls prevention service across CTM, providing falls advice, assessment, and intervention at local venues. The service focuses on both prevention and intervention, building integrated teams and designing a seamless pathway for our patients. You will be valued as an integral part of the service development and supported in developing your skills.
Support Allied Health Professions (AHP) – Physiotherapists and Occupational Therapists – in delivering AHP intervention for patients in the community to prevent and manage falls. Work as part of the CTM Falls Prevention and Management Team to undertake clinical and administrative tasks, liaise with health colleagues, Local Authorities and third sector organisations to ensure care for patients within CTMUHB/ Bridgend/RCT/Merthyr County Boroughs as instructed and delegated by the AHP, responding flexibly to the needs of the service. Assist AHP staff with therapeutic modalities aimed at maintaining patients in the community and preventing avoidable falls.
