NHS Hull, East and North Yorkshire Trust is seeking an Assistant Practitioner to provide early intervention support for children, young people and families. You will deliver packages of care under supervision and work with a wide multidisciplinary team across home, schools and community settings.

The role involves home visits, school visits, and contributing to case discussions under supervision, with a focus on promoting emotional wellbeing and safeguarding.

Formación

• Experience as Allied Health Professional with transferable skills.
• Ability to work with children, young people and families.
• Understanding of safeguarding and care planning.

Responsabilidades

• Provide early intervention support in home and community settings.
• Deliver packages of care under supervision.
• Visit family homes and see clients in schools to advise and support.
• Work with multidisciplinary team to support those with more complex needs.
