A community health provider in Bulwell seeks a skilled assistant practitioner to support the multidisciplinary team with assessments and interventions. The ideal candidate has experience in health and social care, demonstrating the ability to work holistically in a fast-paced environment. They will provide direct patient care, enhance community rehabilitation, and play a vital role in preventing hospital admissions. The position includes ongoing training and development opportunities.
An opportunity has arisen for a highly skilled, enthusiastic, conscientious, experienced and motivated assistant practitioner with a keen interest in the holistic working model, to join the At home clinical team which is part of Urgent Community Response, Pathway 1 and Reablement services.
The service consists of a multi-disciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Registered Mental health nurse, Assistant Practitioners, Rehabilitation Support Workers and administrative support.
The aim of the MDT is to provide assessment and rehabilitation to patients who require urgent intervention to prevent hospital admission, support and assessment to facilitate hospital discharge and further reablement to patient who maybe at risk of deterioration or hospitalisation.
We are a dynamic, developing organisation with well recognised rehabilitation services providing assessment, education, advice, support and rehabilitation for patients within our local community.
The successful candidates will be expected to work holistically alongside the multidisciplinary team which consists of Physiotherapist, occupational therapists, Nurses, Registered mental health nurses and rehabilitation support works. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.
You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.
The ideal candidate would be someone with experience of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients.

Job Purpose
To work alongside Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Support Workers to provide assessment and treatment intervention to people within the Nottingham City Community, who are at risk of hospital admission or to facilitate discharge from hospital.
To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values. You will be working in a variety of settings. You will participate actively in the services training strategy, including in-service training and multi-agency training.
