Dorset Council is recruiting Assessment Support Coordinators to work in East Dorset, Purbeck, North Dorset and Home First Hospital Discharge teams. Roles are hybrid with two days in the office per week, offering full-time and part-time options across multiple locality teams.
You will assess, plan and support people to maximise independence, working with Social Workers, Occupational Therapists and health colleagues. Travel is a requirement.
As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.
Our teams have a strong track record in supporting new starters to Dorset Council Adult Services by being friendly and supportive, developing individuals and offering a detailed and comprehensive induction plan.
