We are seeking a motivated and experienced Assessment and Enablement Officer to join our Adult Social Care service. This is an exciting opportunity to play a key role in supporting adults to achieve greater independence, wellbeing and positive long-term outcomes. Through strengths-based reviews, you will work alongside people with learning disabilities, autism, mental health needs, physical disabilities and older adults to identify progress, respond to changing needs and maximise opportunities for independence and community inclusion. Working across a range of services including home care, supported living, day opportunities, direct payments, residential care and community-based support, you will review existing care and support arrangements to ensure they remain person-centred, outcome-focused and promote prevention, wellbeing and independence. Using your professional judgement and analytical skills, you will work collaboratively with residents, carers, providers, health partners and Adult Social Care colleagues to identify what is working well, overcome barriers to achieving outcomes, and make evidence-based recommendations that support people to live fulfilling lives with the right level of support. This role offers an excellent opportunity to make a meaningful difference to residents' lives while contributing to service improvement and Barnet's vision of enabling people to live healthy, independent and connected lives within their communities.
We are seeking an individual who has significant experience of working with adults with learning disabilities, mental health needs and physical disabilities, and who shares our commitment to delivering high-quality, person-centred services.
