Job summary

Rethink Mental Illness in Durham is seeking an Apprentice Health and Wellbeing Coach to join the Prisons and Criminal Justice Care Navigation team. The role combines on-the-job learning with a Level 2 Diploma, supporting people leaving prison to reconnect with healthcare and improve wellbeing.

You will work with experienced colleagues to provide person-centred support before and after release, while balancing study commitments in a supportive team.

Qualifications

• Educated to Level 2 in English and Maths (or equivalent).
• A confident communicator with good interpersonal skills.
• Able to manage their time effectively and meet deadlines.
• Comfortable using IT systems, including Microsoft Office applications.
• Passionate about supporting vulnerable people and making a difference.

Responsibilities

• Support people leaving prison to access health and wellbeing services in the community.
• Assist with assessments, care planning and wellbeing support activities.
• Work with a range of partner agencies to help individuals achieve their personal goals.
• Support health promotion activities and community-based events.
• Maintain accurate and professional records using organisational systems.
• Develop coaching skills that empower people to manage their own health and wellbeing.
• Work collaboratively with healthcare professionals, criminal justice partners and community organisations.
• Balance your apprenticeship studies with practical experience in a supportive team environment.
