Job Overview
An exciting opportunity has arisen to join an existing nurse led service within a multidisciplinary team based at Bournemouth Hospital but covering Upper Hampshire and Dorset that cares for patients with Liver Disease, including Hepatitis B and C and those experiencing Homelessness.
Location & Duration
Base Location: Royal Bournemouth Hospital. Interview date: 16th December. Secondment duration: 6 months.
Main duties
- Coordinate test results and ensure linkage to care and a smooth care pathway to treatment for any patient diagnosed with Hepatitis B, C or HIV through the project.
- Deliver fibroscanning clinics (with appropriate training) in the Bournemouth and Poole sites.
- Advocate for patients, coordinate care and effectively engage patients with complex needs within the Liver Nursing and Homeless Care team.
Person specification
Essential criteria
- NVQ Level 3 in health and social care management or equivalent.
- 5 GCSEs (to include English & Maths) or relevant experience.
- Experience working with patients who have complex needs, liver disease and Hepatitis C.
- Experience in healthcare settings (hospital, community or joint working with healthcare services).
- Face-to-face communication with patients.
- Experience working with vulnerable adults in a care coordinating, supporting, advisory or advocacy role.
- Ability to document care assessments and progress.
- IT skills: Windows, Word, Excel, PowerPoint.
- Knowledge of assessing and problem solving issues related to care planning.
- Excellent assessment, planning, care coordination and organisational skills.
- Excellent oral and written communication skills, ability to engage with and manage expectations of patients with complex needs.
Desirable criteria
- 1st Level degree, or working towards one.
- Understanding of hospital-based Hepatitis C procedures.
- Training and experience in fibroscanning, frailty assessment and calculations of FRAX and FIB4 scores.
- Experience accessing hospital IT systems, eCamis, EPR.
Technical skills & Competencies
- Documentation of care assessments and progress.
- Use of IT including Windows, Word, Excel, PowerPoint.
- Experience accessing hospital IT systems (eCamis, EPR).
