Qualifications
Essential criteria
- NVQ level 3 : health / social care management or equivalent
- 5 GCSE (to include English & Maths) / or relevant experience
Desirable criteria
- 1st Level Degree, or working towards
Experience
Essential criteria
- Experience of working with patients who have complex needs, liver disease and Hepatitis C
- Experience of or working in healthcare settings, hospital or community or joint working with healthcare services
- Experience of face to face communication with patients
- Experience of working with vulnerable adults in a care coordinating, supporting, advisory or advocacy role
Desirable criteria
- Understanding of Hospital based Hepatitis C procedures
- Trained and experienced in fibroscanning, frailty assessment and to calculate FRAX and FIB4 scores
Technical Skills Competencies
Essential criteria
- Ability to document care assessments / progress
- Use of IT including Windows, Word, Excel, PowerPoint
Desirable criteria
- Experience of accessing hospital IT systems, eCamis, EPR
Knowledge
Essential criteria
- Knowledge of assessing and problem solving issues related to care planning
Interpersonal skills
Essential criteria
- Excellent assessment, planning, care coordination and organisational skills
- Excellent oral and written communication skills, ability to engage with and manage expectations of patients with complex needs, and complex issues in a way that is appropriate for the audience
