Job summary

A regional healthcare provider in Bournemouth seeks an Advocacy Care Coordinator to join their multidisciplinary team. The candidate will coordinate care for patients with liver disease and support initiatives in healthcare settings. Qualifications include NVQ level 3 in health/social care management and experience in coordinating care for vulnerable adults. Advocacy Care Coordinator

An exciting opportunity has arisen to join an existing nurse led service within a multidisciplinary team based at Bournemouth Hospital but covering UHD and Dorset that cares for patients with Liver Disease, including Hepatitis B and C and those experiencing Homelessness.

We require a Care Coordinator / Navigator to work within the project team to coordinate test results and ensuring linkage to care and a smooth care pathway to treatment for any patient diagnosed with Hepatitis B, C or HIV through the project. The role will also involves delivering Fibroscanning Clinics (with appropriate training) in clinic setting on the Bournemouth and Poole sites.

Ideally we would like to recruit a candidate with experience of working with the patient group this service cares for.

We wish to fill this position with a 6 months secondment. Please discuss with your current line manager whether you would be supported in taking up a secondment if successful before applying for this post.

Base Location: Royal Bournemouth Hospital

Interview Date: 16th December 2025

Main duties of the job

The post holder will work with the Liver Nursing and Homeless Care team of nurses to advocate for patients, coordinate care and effectively engage patients with complex needs and in particular treatment for Chronic Hepatitis B and C.

Person Specification
Qualifications
  • NVQ level 3: health / social care management or equivalent
  • 5 GCSE (to include English & Maths)/or relevant experience
  • 1st Level Degree, or working towards
Experience
  • Experience of working with patients who have complex needs, liver disease and Hepatitis C
  • Experience of or working in healthcare settings, hospital or community or joint working with healthcare services
  • Experience of face to face communication with patients
  • Experience of working with vulnerable adults in a care coordinating, supporting, advisory or advocacy role
  • Understanding of Hospital based Hepatitis C procedures
  • Trained and experienced in fibroscanning, frailty assessment and to calculate FRAX and FIB4 scores
  • Ability to document care assessments/progress
  • Use of IT including Windows, Word, Excel, PowerPoint
  • Experience of accessing hospital IT systems, eCamis, EPR
Knowledge
  • Knowledge of assessing and problem solving issues related to care planning
Interpersonal skills
  • Excellent assessment, planning, care coordination and organisational skills
  • Excellent oral and written communication skills, ability to engage with and manage expectations of patients with complex needs, and complex issues in a way that is appropriate for the audience
