Job summary

Warwickshire County Council is seeking a Social Worker for the Return Home Team to oversee and progress care plans for children in care aiming for return home to their families. You will be the allocated worker providing ongoing support, carrying out visits, assessments and reviews with the child’s voice at the centre of planning.

You will work collaboratively with families and partner agencies to assess risk, strengthen families and ensure safe, well-planned return-home arrangements.

Qualifications

• Professional Social Work qualification.
• Social Work England Registration.

Responsibilities

• Hold responsibility for care planning and statutory duties for children in care with reunification plans.
• Develop and review return home plans that are child-centred and evidence-based.
• Provide intensive, relationship-based support to families to strengthen relationships and resilience.
• Coordinate multi-agency planning with education, health and safeguarding partners.
• Monitor progress, adapt interventions and ensure child welfare and safety.
• Maintain high-quality recording, analysis and reporting for decision-making.
