Wiltshire Council is seeking an experienced Advanced Social Worker to join the Rough Sleeper Team in Trowbridge. This permanent, full-time role focuses on mental health support for rough sleepers, bridging social care and mental health services.

You will carry out tailored assessments, care management and multidisciplinary collaboration to secure holistic support and housing outcomes countywide, within a fast-paced environment funded by the Rough Sleeper Initiative.

As a Rough Sleeper Mental Health Advanced Social Worker, you will carry out tailored interventions for rough sleepers with significant mental health challenges, bridging the gap between social care and mental health services.
