A leading healthcare institution in the UK is seeking an experienced Advanced Pharmacist Practitioner to provide leadership in anticoagulation services. You will work closely with clinical teams to ensure patient safety and manage a comprehensive pharmacy service. Candidates should have an independent prescribing qualification and strong leadership skills. The role offers opportunities for development and training within a supportive environment, aiming to enhance the quality of patient care.

We are looking for a dynamic, organised and highly motivated individual to be part of the anticoagulation team and play a key role in anticoagulation stewardship.
You will provide expert leadership, advice and guidance relating to the safe use of anticoagulation, including when patients move between health‑care settings or home. The post‑holder will work with key stakeholders locally, regionally and nationally to deliver patient safety and system‑wide initiatives.
You will support the nursing team to operate nurse‑led clinics but also have an opportunity to develop your own clinics and participate in consultant‑led clinics. An independent prescribing qualification will be required for the role, or a commitment to work towards one.
Previous specialist experience of anticoagulation will be of benefit but is not essential as training can be provided. An insight into managing the risks associated with anticoagulation would be beneficial.
This role is offered as an 8b Advanced Pharmacist Practitioner post; however, there is also the opportunity for you to apply at an 8a level and undertake an agreed development plan to gain the skills and knowledge to progress to an 8b.
