NHS Professionals Limited is recruiting an Advanced Pharmacist for Watford General Hospital. You will support the Lead Pharmacist in delivering clinical services across the Watford site and wider Trust, ensuring safe, effective, economical, and evidence-based medicines use while promoting Trust Formulary compliance.

You will manage medicines information for patients, carers, doctors, nurses, and staff, providing dosing guidance, indications, side effects and treatment options to optimise care.

Formación

• MPharm degree with current GPhC registration.
• MSc or Diploma in Pharmacy Practice or equivalent.
• Experience in clinical pharmacy services, particularly Haematology and Oncology.
• Knowledge of medicines reconciliation, prescription screening and clinical interventions.

Responsabilidades

• Support development and implementation of systems enabling appropriate use of patients' medicines.
• Manage and improve technician-led processes for timely, accurate medicine supply.
• Identify and monitor adverse drug reactions and submit Yellow Card reports.
• Coordinate inpatient and outpatient dispensary operations with the Dispensary Manager.
• Liaise with community pharmacists, GPs and primary care teams for continuity of care.
• Document interventions and escalate medication errors per policy.
