Job summary

Qualifications

• Minimum of one year's experience doing triage in a primary care or urgent care setting.
• Experience of case management and remote triage is highly valued.
• Commitment to evidence-based practice and clinical governance.

Responsibilities

• Triages, assesses, and prioritises online and in-person requests.
• Signposts to services or escalates to urgent or secondary care as needed.
• Conducts home visits and designated clinics for complex health needs.
