Advanced Nurse Practitioner Orpington Primary Care Network

Orpington Primary Care Network (PCN) is seeking a highlymotivated and experienced Advanced Nurse Practitioner (ANP) to join ourdynamic multidisciplinary team. This is an exciting opportunity to be part ofan innovative, patient-focused service committed to delivering high-qualitycare within the local community.

Main duties of the job

Key Responsibilities:

  • Conduct comprehensive clinical assessments and provide expert care to patients
  • Diagnose and manage both acute and chronic conditions independently
  • Prescribe medications within scope of practice and in line with local guidelines
  • Contribute to health promotion, disease prevention and long-term condition management
  • Work as part of a multidisciplinary team across GP practices within the PCN

About the Role: As an ANP within Orpington PCN, you will work autonomously and collaboratively across our MDT to assess, diagnose, treat, and manage patients with a range of conditions across our member practices. You will play a key role in supporting primary care services, reducing GP workload, and improving patient access and experience.

Job responsibilities

Clinical Duties:

Conduct advanced assessments, diagnose and manage patients with a wide range of conditions.

Provide same-day/urgent care consultations within practices or at hub sites.

Prescribe medications within scope of practice.

Manage long-term conditions (e.g., diabetes, COPD, hypertension) in line with evidence-based practice.

Request, interpret and act on diagnostic investigations such as blood tests, ECGs, X-rays, etc.

Undertake home visits and care home rounds where appropriate.

Leadership and Collaboration:

Support the development of clinical pathways and integrated models of care.

Contribute to service development initiatives aligned with PCN priorities (e.g., enhanced access, anticipatory care).

Supervise and support junior clinical staff or trainees where needed.

Documentation and Compliance:

Maintain accurate and up-to-date clinical records in line with GMC/NMC standards.

Participate in clinical audit, quality improvement projects, and appraisals.

Adhere to safeguarding, infection control, and data protection policies.

Person Specification
Experience
  • Minimum of 3 years post-qualification experience working in a primary care or urgent care setting.
  • Competence in clinical assessment, diagnostics, prescribing, and decision-making.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and within a team.
  • Experience working within a PCN or multidisciplinary team environment.
  • Training in chronic disease management or minor illness/injury.
  • Experience in digital consultations (e.g., AccuRx, eConsult).
  • Experience using EMIS
  • Knowledge of QOF and Enhanced Services.
  • Experience in working in an acute illness hub
Qualifications
  • Registered Nurse with valid NMC registration.
  • Completed MSc or PGDip in Advanced Clinical Practice (or equivalent recognised ANP qualification)
  • Independent Non-Medical Prescriber (V300)
  • Experience working within a PCN or multidisciplinary team environment.
  • Training in chronic disease management or minor illness/injury.
  • Experience in digital consultations (e.g., AccuRx, eConsult).
  • Experience with EMIS
  • Knowledge of QOF and Enhanced Services.
  • Experience in working in an acute illness hub
