A healthcare organization in the United Kingdom is seeking an Advanced Nurse Practitioner to provide high-quality clinical care for housebound patients. Offering a competitive salary of up to £51,700, the role involves working collaboratively with local GP practices to deliver enhanced primary care services.

Norfolk Primary Care (NPC) is an established Primary Care provider of clinical services. We are a key provider of Primary Care services in the Norfolk & Waveney system working with the local GP practices.
NPC has been sub-contracted to provide a home visiting service to the Practices in the Norwich PCNs. This comprises of a team of clinicians who are GPs, Advanced Nurse Practitioners, qualified nurses with a prescribing qualification (or working towards), and Advanced Clinical Practitioners working across the Norwich PCNs. The staff provides additional Primary Care support to that currently being provided by individual practices.
Supported by a Service and Clinical Lead, the HVS team are responsible for working with all HVSparticipating Practices focusing on proactive support to patients residing at home (including care homes).
The aim of the HVS is to reduce the number of avoidable admissions into hospital and to deliver more consistent, enhanced services from Primary Care. It offers proactive health care support to housebound patients, alongside the general population of patients who may require home visiting services.
This exciting role will provide a high quality service to housebound patients on behalf of their registered GP Practice, and the wider PCN.
You will be an experienced clinician who will be acting within their professional boundaries and competencies. You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and follow up of care.
You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for the patients within Primary Care. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the GP Practices to meet the needs of patients in supporting the delivery of policy and procedures, and providing nurse leadership if / as required.
In order to work at this level, those registered with the NMC / HCPC, requirements for Advanced Practice must be met.
