Job summary

Qualifications

• Advanced Nurse Practitioner with autonomous clinical scope.
• Experience delivering care in community/home settings.
• Prescribing and decision making for complex health needs.

Responsibilities

• Provide advanced clinical assessment of patients in the community.
• Make clinical decisions and prescribe where appropriate.
• Manage frailty and long-term conditions within home care.
• Support early intervention and hospital avoidance strategies.
• Collaborate with district nursing, GP practices and wider MDT.
