Job summary

Burnley West PCN is seeking an Advanced Nurse Practitioner to deliver expert clinical care across its patient population in Burnley. You will maintain up-to-date knowledge, support complex decision-making, and lead focused clinical activities within the practice team.

You'll develop treatment plans, participate in audits and safeguarding, and mentor junior staff while working within professional codes and governance frameworks to ensure high-quality care and continuous improvement.

Responsibilities

• Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level
• Assess effectiveness of own clinical practice and care delivery through self-reflection and peer review
• Promote evidence-based practice through latest guidelines and development of practice based research
• Participate in continuing professional development opportunities to ensure up to date evidence-based knowledge
• Develop and maintain a Personal Learning Plan and maintain their professional registration
• Record accurate consultation data in patient records in accordance with guidance and standards
• Work collaboratively with Locality team to consider impact and strategies for implementation
• Participate in multi-disciplinary protocol and clinical pathways and audits
• Practice in compliance with Code of Professional Conduct and within scope of practice
• Demonstrate critical understanding of expanded responsibility and autonomy
• Know when to seek help with clinical judgement and reflect on practice and learning
• Work with individuals, families and carers using range of assessment methods
• Demonstrate effective communication skills to support decisions and planning of care
• Use expertise and decision-making to inform clinical reasoning
• Initiate, evaluate and modify interventions including prescribing medicines and lifestyle advice
• Manage patients with a range of acute and chronic conditions, providing expert advice
• Identify and support patients at risk of long-term conditions
• Be a professional role model and actively engage in peer review
• Seek feedback from individuals and communities for service improvements
• Develop an area of specialist interest and lead within the organisation
• Review clinical practices in response to national policies and initiatives
• Develop practice protocols in line with needs of the patient and legislation
