Job summary

One Health Lewisham Limited seeks an Advanced Nurse Practitioner to join its Home Visiting Service in Lewisham, with coverage across Lewisham and potentially Lambeth. Qualifications

• Registered Nurse with full NMC registration.
• MSc in Advanced Clinical Practice (or equivalent).
• Experience working in General Practice (minimum 1 year).
• Strong clinical assessment, diagnostic, and decision-making skills.
• Ability to work autonomously and lead clinically.
• Independent Prescriber qualification.
• Car owner with business insurance cover and a full clean driving licence.

Responsibilities

• Deliver high-quality, patient-centered care to at least 8 housebound frailty patients per day, in their own homes, or patients housed in care homes during regular ward rounds
• Independently assess, diagnose, and manage patients with complex and undifferentiated conditions
• Develop and implement treatment plans, including prescribing where appropriate
• Work collaboratively with multidisciplinary teams to ensure seamless patient care
• Provide clinical leadership and support to colleagues
• Contribute to service development, audits, and quality improvement initiatives

Location: Lewisham (Home Visiting Service)
Hours: Full-Time, 37.5 hours per week
Salary: £65,000.00
Contract: Permanent

The Role

We are looking for an experienced and motivated Advanced Nurse Practitioner (ANP) to join our Home Visiting Service, covering the Lewisham borough. You will also be deployed across our care homes, in Lewisham and/or Lambeth, giving you a varied and challenging workload under the expert supervision of our Clinical Leads of the respective services, as well as our Medical Director.

In this autonomous and rewarding role, you will:

What We're Looking For

Essential

• Registered Nurse with full NMC registration
• MSc in Advanced Clinical Practice (or equivalent)
• Experience working in General Practice (minimum 1 year)
• Strong clinical assessment, diagnostic, and decision-making skills
• Ability to work autonomously and lead clinically
• Experience with EMIS clinical system
• Excellent communication and interpersonal skills
• Independent Prescriber qualification
• Car owner with business insurance cover and a full clean driving licence, as this role requires driving to patient homes. Acute home visit parking permit will be provided

Desirable

• Background in leadership, service development, or quality improvement
• Experience working within community or integrated care settings
