Job description
Overview

Advanced Clinical Prescribing Practitioner to add to our urgent care team. The successful candidate will be working alongside our ANPs, ACP and AHPs to provide urgent care to our population for 37,000 patients. The team will be led by a GP on site, and supported by the wider team across Hamble Valley Health.

Responsibilities
  • Duties will include telephone and face to face assessments of patients who have been signposted to an appointment at the Urgent Care Centre by our clinical team. You will work as part of a larger team, supported by a GP lead.

Job Description / Responsibilities
  • Main Purpose: To be responsible for independently leading and managing practice, using an advanced level of professional accountability, autonomy and judgement for highly complex decision-making across a broad range of situations/conditions.
  • To work autonomously, but as part of the Duty Team, using advanced knowledge and skills to holistically assess the identified patient caseload and conditions.
  • To plan and manage complete episodes of care, delegating and referring as appropriate, to optimise health outcomes and resource use, and provide direct support to patients and carers.
  • To promote service and policy development and redesign informed by best practice and evidence-base, in accordance with values-based care.

  • Essential – Professional registration with relevant professional body
