We are seeking a part‑time independently prescribing Advanced Nurse Practitioner or Advanced Clinical Practitioner. The ideal candidate is proactive, enthusiastic and enjoys working as part of an outstanding, long‑established team.

Main duties of the job

• Work as an autonomous practitioner as part of a multi‑professional team in the delivery of high quality care to a registered patient list of approximately 10,900 patients.
• Triage, assess, diagnose and treat patients requesting same‑day appointments, or refer to the GP/nursing team for further intervention/management.
• Manage own caseload (prescribing qualifications essential).
• Act as a clinical expert in direct contact care.
• Facilitate expert inter‑professional care for the ill patient and their dependants by direction, supervision and example; maintain all records.

Job Summary

Work as an autonomous practitioner as part of a multi‑professional team, triage, assess, diagnose, treat and manage patients on the primary care register. The role includes managing own caseload, providing direct contact care, and maintaining accurate electronic and written patient records.

Key Responsibilities

• Assess, diagnose, plan, implement and evaluate treatment/interventions.
• Diagnose and manage both acute and chronic conditions.
• Clinically examine and assess patient needs and plan clinical care accordingly.
• Care for patients presenting with an undifferentiated diagnosis and refer appropriately.
• Support patients to adopt health promotion strategies that promote healthy lifestyles.
• Demonstrate appropriate communication, ensuring patients are fully informed and give consent to treatment.

The Ideal Candidate Will Also Have:

• Autonomous practitioner experience in primary care and minor illness.
• Ability to prescribe independently; at least 2 years as an Advanced Nurse Practitioner within general practice.
• Minor Illness and Injury qualifications.
• Knowledge of SystmOne and Clarity Teamnet is an advantage; a fast learner who can acquire IT skills quickly.
• Strong communication, empathy, and negotiation skills; ability to handle highly distressing or emotional circumstances.
