Centraland South Primary Care Network is seeking two highly skilled and motivated AdvancedNurse Practitioners (ANP) or Advanced Clinical Practitioners (ACP) to join our expanding multidisciplinary team.

Theseare ARRS-funded roles working across Park House Medical Centreand Tarbock Medical Centre, offering the opportunity to deliverhigh-quality, autonomous clinical care within supportive and well-establishedgeneral practice environments.

Weare looking for practitioners with a broad scope of practice and anappropriate personal formulary, confident in managing undifferentiatedpresentations and working independently as autonomous clinicians.

Thesuccessful candidates will be employed by Roby Medical Centre on behalfof Central and South PCN.

Main duties of the job

Youwill work autonomously to assess, diagnose, and manage a range of patients,including those with complex or long-term conditions. You Will demonstratesound clinical judgement, evidence-based practice, and a patient-centredapproach in all aspects of care.

Youwill contribute to the PCNs wider clinical network, supporting safe,effective, and sustainable primary care delivery. Both roles are designed forexperienced ANPs or ACPs who meet the ARRS eligibility criteria and are readyto work independently within a supportive, structured environment.

Job responsibilities

Key Responsibilities

  • Clinical Expertise
  • Independently assess, diagnose, plan, and implement care for patients across the lifespan, including those with chronic disease management needs.
  • Provide evidence-based clinical interventions and prescribe medication as appropriate under independent prescribing rights and in line with current guidance Pan Mersey, and NICE guidance.
  • Manage patients presenting with undifferentiated and undiagnosed problems, ensuring care aligns with NHS England (NHSE), Care Quality Commission (CQC), and Royal College of Nursing (RCN) standards.
  • Autonomous Practice
  • Work as an autonomous practitioner, taking accountability for clinical decision-making and patient outcomes.
  • Undertake clinical risk assessment, ensuring care delivery adheres to national guidance, including NICE and NHSE protocols.
  • Chronic Disease Management
  • Deliver high-quality care for patients with chronic conditions such as diabetes, asthma, and hypertension.
  • Promote patient self-management and provide education to improve long-term health outcomes.
  • Quality and Governance
  • Ensure all practice aligns with CQC standards and contribute to audits, quality improvement initiatives, and governance processes within the practice.
  • Maintain accurate and contemporaneous records in line with CQC and professional standards.
  • Continuing Professional Development (CPD)
  • Take ownership and responsibility for your professional development, remaining up to date with relevant guidelines, including RCN and NHSE updates.
  • Participate in annual appraisals with a named appraiser and maintain a professional portfolio to evidence CPD and reflective practice.
  • Collaboration and Leadership
  • Support the development of the multidisciplinary team, sharing expertise and providing mentorship where required.
  • Contribute to the practices goals of increasing capacity and accessibility to care through the ARRS scheme.
Person Specification
Experience
  • Experience in chronic disease management and managing undifferentiated presentations.

  • Experience supporting or mentoring junior staff.
Qualifications
