The successful candidate would have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients.
The successful candidate will practiceautonomously, within their speciality and be able to provide patient-centredcare. This will encompass the skills of clinical assessment, examination, diagnosis,and treatment within an agreed scope of practice.
Be involved in decision-making and support newways of working that create efficient and patient focused services.
Support the ongoingdevelopment of clinical practice and standards of care within the service,including the development of policies, procedures, protocols, and guidelines incollaboration with multi-disciplinary colleagues.
Administration requirements to deliver safepatient care and be involved in the continuity of care.
Involvement in signposting, triage, and seeingpatients.
Demonstrate a high level of expertise, providing advice, education, and support to patients, their families, and their carers.
Thornaby and Barwick Medical Group is part of BYTES PCN and provide services to patients via a multi-disciplinary team of GPs, Advanced Practitioner, Nurses, GPAs, Physician Associate, Pharmacy Technician, MSK, Social Prescribers, Adult and children mental health professionals, alongside competent and efficient admin and reception teams and managers.
We have 6 GP Partners and 7 salaried GPs.
Our patient population is around 21,000, working across 2 practice sites.
We are a training practice and regularly host GP Trainees and hold education meetings to develop shared learning.
Job description and responsibilities
The Advanced Nurse Practitioner (ANP) hasbeen assessed and deemed as competent in practice using their expert clinicalknowledge and skills. They have the freedom and authority to act and makeautonomous decisions in the assessment, diagnosis, and treatment of patients.
The post holder works at the level of advancedclinical practice and has developed and can evidence the underpinningcompetencies applicable to the specialty or subject area, i.e., the knowledge, skills,and behaviours appropriate to the healthcare professionals (HCP) setting andjob role.
The role provides an opportunity for patients toavoid the need to be referred to a GP, thereby enabling them to receive timelycare and negate unnecessary delay in receiving treatment.
An ANP is autonomous in making decisions basedon assessment, diagnosis and interpretation of test results and will initiate, evaluate,and modify a range of interventions which may include prescribing medicines,therapies, lifestyle advice and care.
The following are the core responsibilities of theAdvanced Nurse Practitioner.
There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffing levels,however core responsibilities include, but are not limited to:
Practice in compliance with their respectivecode of professional conduct and within their scope of practice, beingresponsible and accountable for their decisions, actions, and omissions at thislevel of practice
Demonstrate a critical understanding of theirbroadened level of responsibility and autonomy and the limits of own competenceand professional scope of practice, including when working with complexity,risk, uncertainty, and incomplete information
Act on professional judgement about when to seekhelp, demonstrating critical reflection on own practice, self-awareness,emotional intelligence, and openness to change
Work in partnership with individuals, families,and carers, using a range of assessment methods as appropriate
Demonstrate effective communication skills,supporting people in making decisions, planning care, or seeking to make apositive change
Use expertise and decision-making skills toinform clinical reasoning
Initiate, evaluate, and modify a range ofinterventions which may include prescribing medicines, therapies, lifestyleadvice and care
Exercise professional judgement to manage risk
Manage patients presenting with a range of acuteand chronic medical conditions, providing subject matter expert advice
Implement and evaluate individual specialisedtreatment plans for chronic disease patients
Identify, manage, and support patients at riskof developing long-term conditions, preventing adverse effects to the patients'health
Be a professional role model for the organisation
Actively engage in peer review to inform own andothers practice, formulating and implementing strategies to act on learning andmake improvements
Actively seek feedback and involvement fromindividuals, families, carers, communities, and colleagues in the co-productionof service improvements
Critically assess and address own learningneeds, negotiating a personal development plan that reflects the breadth ofongoing professional development across the four pillars of advanced clinicalpractice (clinical practice, leadership and management, education andresearch).
We may require the role to provide chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequired
Maintain accurate clinical records inconjunction with extant legislation and to ensure SNOMED CT codes are usedeffectively
Critically appraise and synthesise the outcomeof relevant research, evaluation and audit, using the results to underpin ownpractice and to inform that of others
Contribute to practice targets (QOF, etc.), complying with local andregional guidance
Chaperone patients where necessary
Support the team in dealing with clinicalemergencies
Support the team with all safeguarding matters inaccordance with local and national policies
Understand practice and local policies forsubstance abuse and addictive behaviour, referring patients appropriately
Deliver opportunistic health promotion whereappropriate
Conduct home visits
Carry out care home visits when required
You may be expected to partake in practice meetings to develop new pathways, contribute to discussions and understand practice updates to policies or procedures
Secondary responsibilities
In addition to the primary responsibilities, the AdvancedNurse Practitioner may be requested to:
Support practice and PCN leads to ensurecompliance, and undertaking audits as necessary
Monitor and ensure the safe storage, rotation,and disposal of medicines
Participate in local initiatives to enhanceservice delivery and patient care
Support and participate in shared learningwithin the organisation
Continually review clinical practices,responding to national policies and initiatives where appropriate
Participate in the review of significant andnear-miss events applying a structured approach i.e., root cause analysis (RCA)
Commensurate with their role, support thedevelopment of clinical services within the organisation, liaising withexternal agencies and professional organisations as required
