Job description

We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our Team. This role is pivotal in providing high-quality, patient‑centred care in a fast‑paced, supportive general practice environment.

As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision‑making, and prescribing, with a commitment to upholding the standards of care and safety expected by Mayflower Medical Group.

Main duties of the job

Key responsibilities include the applicationof advanced clinical skills to assess, diagnose, treat, and advise patientsface to face, ensuring the use of evidence‑based practice. The role encompasses the following areas:

  • Undifferentiated Patient Care within an AM and PM session
  • Patient follow up - respond to previous case management
  • AutonomousPractice - as per NICE guidelines
  • Case Management - Ensuring pathways meet the needs of our service

Job responsibilities
Clinical Responsibilities

Deliveradvanced and complex clinical care, providing assessment, diagnosis, treatment,and prescribing for patients with acute, chronic, and complex conditions.

AutonomousPractice

Work independently within the scope of practice to manage completeclinical episodes, using advanced diagnostic skills and clinical judgment.

Case Management

Manage patient cases from initial assessment to conclusion, includingfollow-ups and referrals, ensuring continuity of care.

Prescribing

Utilise prescriptive authority responsibly following current guidelines andprotocols to enhance therapeutic outcomes for patients.

Home visiting

undertake home visiting as and when required, including triage of home visits.

Leadership and Guidance

Provide mentorship and supervision to nursing staff, trainee advancedpractitioners, and other healthcare professionals to enhance team competencies.

ProfessionalDevelopment

Engage in continuous professional development to keep abreast ofmedical advancements and to further develop clinical skills.

Experience
  • Extensive experience in managing complete episodes of clinical care including assessment, diagnosis, treatments, and prescribing
  • Advanced clinical skills in a primary care setting, with proficiency in using clinical diagnostic tools and conducting complex clinical procedures
Qualifications
  • Masters degree in advanced Practice, or equivalent post‑graduate qualification, or full accreditation or eportfolio
  • Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate
  • Further qualifications in a specialist area such as diabetes, dermatology, or emergency care
  • Experience and competency in minor surgery procedures or chronic disease management
