A healthcare practice in Lincolnshire is seeking an experienced Advanced Nurse Practitioner to join their collaborative team. We aim to provide the best working environment, involving our staff to choose the best way of providing care to patients - flexible appointment length; face to face or remote appointment types. We are also happy to be flexible around routine start and finish times. The number of sessions and days worked are also flexible and negotiable.
You will work in a supportive, friendly environment with our integrated team of Doctors, ANP's, Clinical Pharmacist, Paramedic, Social Prescriber, Care Co-ordinator, Social Prescriber, Mental Health Nurse, Specialist Diabetes Nurse and Nursing team.

Abbeyview Surgery is situated in the semi rural town of Crowland serving circa 9300 patients.
This is an exciting opportunity to join our friendly team of dedicated professionals to deliver high quality, patient centred care to our patients.
In the past, our innovative team have held patient education events, supported the opening of dementia cafe, attended the local school to teach children, We work as a team, and with our PPG to identify ways to improve services for patients of the Practice. The successful applicant will be integral in helping us to identify and further improve services for our patients.
Main duties of the job
To support and work alongside the GPs and clinical team to deal with a variety of long standing conditions, as wells as acute/on-the-day patients including triage. This role includes assessment, diagnosis and treatment of these patients who present with undiagnosed conditions. Patient contacts may be face to face, over the telephone or electronic.
The post holder will provide and maintain a high standard of care for patients as well as providing support to the GPs and other members of the primary health care team.
This position may very occasionally also include visiting housebound patients to assess, diagnose and treat when they are unable to access their practice.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
