Advanced Nurse Practitioner

Advanced Nurse/Clinical Practitioner – Opportunity available for an Advanced Nurse Practitioner to work across the Primary Care Home Visiting Service, based in Nottingham. The team sits within a Primary Care Service and the role is predominantly responsible for the delivery of evidence-based services, working as part of a multidisciplinary team and providing care within the scope of practice to the entitled patient population.

Responsibilities
  • Deliver evidence-based services autonomously, focusing on health promotion, chronic disease management, health prevention and wellbeing programmes.
  • Provide well man clinics and support the management team in reviewing clinical policy and procedure.
  • Develop, implement and embed health promotion and wellbeing programmes; develop, implement and evaluate individual treatment plans for acute and non-acute conditions.
  • Support the clinical team in developing treatment plans for chronic disease patients and identify, manage and support patients at risk of developing long‑term conditions.
  • Prescribe medication in accordance with PGD and within own scope of practice and review medication for effectiveness, need and national guidance.
  • Deliver routine nursing care to patients as required, following clinical evidence, NICE and the NSF.
