Job summary

A healthcare provider in North East England is seeking a highly motivated Advanced Nurse Practitioner for an 18-hour role over two days. The successful candidate will deliver autonomous clinical care, support a multidisciplinary team, and contribute to ongoing service development. Job description

We are looking for a highly motivated experienced ANP to join our Team at Park Lane Practice. This post is for 18 hours over two days with overtime.

The successful candidate will play a key role in delivering autonomous clinical care, supporting our multidisciplinary team, and contributing to ongoing service development as we continue to expand our patient list.

Main duties of the job

Provide high-quality clinical assessment, diagnosis, treatment, and management of patients presenting with a wide range of undifferentiated and undiagnosed conditions.

Undertake acute illness / disease assessment and management, including prescribing and reviewing medications within scope of practice.

Support urgent on-the-day demand alongside the duty team.

Deliver proactive, patient-centred care for long-term conditions in collaboration with the wider clinical team.

Request and interpret investigations, make referrals, and liaise with other health and social care professionals as appropriate.

Contribute to clinical governance, quality improvement, and the development of care pathways.

About us

We are looking for a highly motivated experienced ANP to join our Team at Park Lane Practice. Our Practice is made up of 4500 patients and a team of 2 x GPs, 1 x ANPs, 1 x Practice Nurses, 1 x Health Care Assistant, 1 x General Practice Assistant and Reception team.

Job responsibilities

Key responsibilities:

Undertake patient consultations in surgery, and home visiting when required.

Work independently to assess, diagnose, plan, implement, and evaluate treatment plans within scope.

Prescribe and review medication in accordance with local and national guidelines.

Identify, manage, and refer patients appropriately, ensuring safe and effective clinical decision-making.

Maintain accurate and contemporaneous patient records.

Participate in clinical meetings and contribute to the ongoing development of practice services.
