An Advanced Nurse Practitioner post at Amersham Vale Practice, a training practice of around 16,000 patients. The core is the duty desk and triage service: assessing online requests, prioritising need and deciding on remote or face-to-face care, signposting or escalation. The post also includes designated clinics and home visits within a practice that runs a homeless clinic and a drug and alcohol service. Fit depends on independent prescribing, triage experience and the ability to see children and patients with mental health needs.
