Experience of working in Primary Care and a prescribing qualification is essential for this role.

The Advanced Nurse Practitioner will be expected to undertake all duties andresponsibilities associated with an Advanced Nurse Practitioner working withinPrimary Care, including (but not limited to):
Work autonomously as an advanced practitioner with an extended scope of practice, contributing to the delivery of comprehensive Primary Care services.
Conduct face-to-face consultations, telephone assessments, and occasional home visits, providing accessible and responsive care to our patients.
Perform clinical assessments, take patient histories, formulate diagnoses, and develop evidence-based treatment and management plans.
Prescribe independently within the scope of professional practice, ensuring safe and effective patient care.
Maintain accurate, detailed, and contemporaneous clinical records that clearly document assessment, decision-making, and interventions.
Collaborate closely with our multi-disciplinary team, sharing clinical expertise and contributing to a culture of continuous learning and excellence.
Ensure all clinical practice aligns with the latest evidence-based guidelines, supporting a modern, prudent, and patient-centred approach to Primary Care.
We are a well-established, thriving practice operating across two sites, proudly serving a rural community of over 13,280 patients. Our team includes six GP partners, salaried and retained GPs, ANPs, a multiskilled nursing team, pharmacy, wellbeing team, and mental health nurse all supported by an exceptional admin team.
