Advanced Nurse Practitioner (ANP)

Location: Epsom
Contract: Permanent, Full‑Time (5 days per week)
Hours: 3 days on the road | 2 days on site
Salary: Competitive, dependent on experience

A PCN in Epsom is seeking a highly skilled and motivated Advanced Nurse Practitioner (ANP) to join its dynamic multidisciplinary team. This is an exciting opportunity for an experienced clinician to play a key role in delivering high‑quality, patient‑centred care across the Eomsom community.

Working three days on the road and two days on site, the ANP will provide flexible, responsive care to patients in a variety of settings, including home visits, care homes, and community clinics. The successful candidate will work autonomously, undertaking assessments, diagnostics, and prescribing, as well as supporting chronic disease management and urgent care needs.

Key Responsibilities:
  • Deliver comprehensive clinical assessments, diagnoses, and treatment plans for patients across community and practice settings.
  • Provide acute and chronic disease management, including prescribing and medication reviews.
  • Work collaboratively with GPs, nurses, paramedics, pharmacists, and social care teams to deliver integrated, patient‑focused care.
  • Support triage and same‑day access services to improve patient outcomes and reduce hospital admissions.
  • Contribute to service development and quality improvement initiatives within the GCN.
Essential Requirements:
  • Registered Nurse with current NMC registration.
  • Master’s level qualification in Advanced Clinical Practice (or equivalent).
  • Independent prescribing qualification (V300).
  • Proven experience working autonomously within primary or community care settings.
  • Excellent clinical reasoning, communication, and decision‑making skills.

  • Experience working in home visiting or community‑based roles.
  • Confidence managing complex long‑term conditions and acute presentations.
