Job summary

NELFT NHS Foundation Trust invites applications for a Band 7 Advanced Diabetes Specialist Nurse (Part Time, 28.5 hours per week) covering Basildon, Billericay, Brentwood, Wickford and Thurrock. The post supports community diabetes teams and home-based care within an integrated service.

The successful candidate will hold a postgraduate diabetes qualification or equivalent experience, demonstrate strong patient education skills and be committed to CPD, with travel across multiple sites as required.

An exciting opportunity has arisen for a part-time Band 7 Advanced Diabetes Specialist Nurse to join our Integrated Diabetes Nursing Service, covering Basildon, Billericay, Brentwood, Wickford and Thurrock.

We are looking for an enthusiastic and motivated individual with excellent communication skills who can work autonomously, as part of a multidisciplinary team, and contribute to the wider Diabetes Service in supporting people with the complexities of diabetes management.

The successful candidate will have:

• A strong foundation of diabetes knowledge and experience in diabetes specialist care, ideally with at least two years' experience.
• A postgraduate qualification in Diabetes or a minimum of five years' equivalent experience.
• Experience in delivering patient structured education and teaching.
• Good IT skills and an interest in developing knowledge and skills in diabetes technology.
• Excellent communication and interpersonal skills.
• A commitment to continuing professional development.
• The ability to work effectively across multiple locations and travel between the boroughs as required.

Detailed job description and main responsibilities

• The post holder will need to have a clear understanding of the challenges faced by primary and secondary care in meeting the recommendations for the development of heart failure services, such as those contained in the National Services Framework for Diabetes.
• To undertake a holistic, unified assessment process consistent with the approach to Single Assessment in partnership with all agencies, to ensure that acute admission is prevented where possible and that this approach identifies and delivers services to meet and sustain patient’s needs.
• To keep accurate up to date records of patients defined within the case finding process and communicate timely and informative written reports and summaries for other agencies e.g. GP’s, Consultants, MDTs and Social Services.
• To establish robust communication networks with patients, their carer’s, other members of the multi-disciplinary team as well as other agencies, and the whole health economy involved in the care of the patients.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period

This post will be subject to a probationary period.

High Cost Area Supplement – Fringe (Basildon, Thurrock, Brentwood, Harlow and Dartford only)

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,346 to a maximum of £2,270 p.a. pro rata for part time)
