University Hospitals Dorset (UHD) is seeking an Advanced Clinical Practitioner (ACP) to join our forward-thinking Stroke and Neuro Service, working across our Stroke Unit and our Early Supported Discharge (ESD) Teams -- a leading service that transforms patient recovery by delivering specialist care at home.

Main duties of the job

The post holder will contribute to both the Stroke Unit and the Early Supported Discharge Team, with duties divided on a 60:40 basis, supporting seamless patient care across the stroke pathway.

• HCPC Registered AHP
• Extensive post graduate clinical stroke/neuro experience in an acute/sub acute stroke team.
• Management and or Leadership course relevant to healthcare environment.
• Supplementary or independent prescriber.

Shortlisting and interview

• Demonstrable experience of managing patient's care at an advanced level of practice with specific knowledge and skills in a field of expertise within the stroke and neuro rehab context.
• Publication/Conference experience.
