University Hospitals Dorset (UHD) is seeking an Advanced Clinical Practitioner (ACP) to join our forward-thinking Stroke and Neuro Service, working across our Stroke Unit and our Early Supported Discharge (ESD) Teams -- a leading service that transforms patient recovery by delivering specialist care at home.

The Candidate Were Looking For:
The post holder will contribute to both the Stroke Unit and the Early Supported Discharge Team, with duties divided on a 60:40 basis, supporting seamless patient care across the stroke pathway.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
