Job summary

Torbay and South Devon NHS Foundation Trust is launching an Advanced Clinical Practitioner role within a Healthy Ageing Frailty / SDEC model at Newton Abbot Community Hospital. The ACP will work autonomously across clinical, leadership, education and research pillars to deliver safe, person-centred care for older adults with frailty.

The post requires a Master's in Advanced Clinical Practice (or NHSE portfolio) and registration with NMC/HCPC, with evidence of research and service improvement

Qualifications

• Degree or diploma in the core professional area.
• Current registration with a relevant professional body (NMC/HCPC).
• Master's degree in Advanced Clinical Practice or NHSE portfolio route.
• Leadership/management qualification or equivalent experience.
• Evidence of active involvement in research and portfolio development.
• Experience leading service improvement projects in health care.
• Immediate or Advanced Life Support qualification.

Responsibilities

• Deliver timely same-day assessments and interventions to prevent unnecessary hospital admissions.
• Coordinate transitions between SDEC, Virtual Ward, community teams and primary care.
• Provide visible clinical leadership during daily huddles and MDT discussions.
• Support teaching, coaching and supervision; develop ACP capabilities.
• Lead or contribute to audits, evaluations and quality improvement projects.
• Work with high professional autonomy within defined scope and governance.
