Job summary

Rosedean House Surgery is seeking an Advanced Clinical Practitioner to join the primary care team. The role focuses on autonomous assessment, diagnosis and treatment across full episodes of care, handling minor illness and long-term conditions, with governance duties alongside the Lead Nurse.

The post includes mentoring, leadership within a multidisciplinary team, and opportunities to transform care pathways.

Qualifications

• MSc in Advanced Practice or equivalent Level 7 qualification.
• Independent Prescriber qualification (V300).
• Current NMC, HCPC or GPhC registration.

Responsibilities

• Make independent decisions on assessment, diagnosis, treatment and management across full episodes of care.
• Manage minor illness and long-term conditions.
• Drive clinical governance with the Lead Nurse to ensure safe, high-quality service delivery.
• Mentor team members and share skills across professional boundaries.
• Help transform and modernise care pathways within the multidisciplinary team.
