Job summary

Locum Ready is seeking an Advanced Clinical Practitioner to support regional community health and primary care services in Crofton. This role offers flexible sessional work in community clinics, ideal for ACPs who can work autonomously and handle urgent patient demand.

You will join established clinical teams providing advanced decision-making, routine assessments and urgent care support across structured service environments, with flexible shifts including weekdays and evenings.

Qualifications

• ACP with Level 7 qualification required.
• Clinical background in Nursing, Paramedic Science or similar.
• Independent Prescribing (V300) desirable.
• Experience in GP, urgent care or community health.
• Ability to work independently in varied clinical environments.
• Enhanced DBS.

Responsibilities

• Deliver ACP-led sessions within community and primary care settings.
• Manage acute, undifferentiated and minor illness presentations.
• Support urgent patient demand via telephone or face-to-face triage.
• Prescribing safely within scope of competence.
• Managing clinical actions including results, medication reviews and follow-up tasks.
• Recording accurate and clear digital documentation.
