Bury GP Federation is seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to lead our Acute Visiting Service (AVS) team across the Bury Primary Care Networks (PCNs). This is an exciting opportunity for a senior clinician to combine advanced clinical practice, leadership, education, and service development in a dynamic and supportive environment. You will play a key role in shaping the delivery of high‑quality, responsive care to patients in their own homes and local communities. As the AVS Team Lead, you will manage and mentor a growing multidisciplinary team, support the professional development of colleagues, and drive continuous improvement across the service. Provide autonomous, high‑quality clinical care including same‑day and scheduled consultations, home visits, and specialist clinics.
Lead, support and manage the AVS team, undertaking appraisals and supporting ongoing professional development.
Represent Bury GP Federation at locality and regional meetings, promoting collaboration across the health and care system.
Deliver clinical supervision and act as a prescribing or clinical supervisor for trainee ACPs and other clinicians.
Lead on service development, governance, and quality improvement projects.
Support research, audit, and evaluation activities within the service.
Promote equality, diversity, and inclusion in all aspects of practice.
