Advanced Clinical Practitioner (ACP) - Raven PCN

The ACP will act within professional boundaries, will provide care for the patients, from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care.
The candidate will have excellent leadership and clinical skills, as well as evidence of continuing professional development. The candidate will have drive, ambition, passion, a flexible approach and be dedicated to working in a fast paced.

The ACP will have the support of all colleagues and management throughout their time with us and most importantly when settling in.
Main duties of the job
The ACP will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practices. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC/HCPC requirements for advanced practice must be met.
About us
RAVEN Primary Care Network (PCN) is made up of the following five GP practices along with their branch surgeries; Stag Medical Centre, Gateway Primary Care, Brinsworth Medical Centre, Treeton Medical Centre and Thorpe Hesley Surgery.
The successful candidate would be expected to work across all practices along with the existing team.
RAVEN PCN currently services around 45,000 patients, working collaboratively with GP practices, other health care professionals and local services to improve health outcomes for our patient population.
The PCN team is made up of a mix of clinical and non-clinical employees, supported by the five member GP practices.
Our geographical location is spread across Rotherham and our patient demographic is variable from practice to practice.
Job responsibilities
Key responsibilities Clinical practice
- Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis.
- Clinically examineand assess patient needs from a physiological and psychological perspective,and plan clinical care accordingly.
- Assess, diagnose,plan, implement and evaluate interventions/treatments for patients with complexneeds.
- Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions,integrating both drug- and non-drug-based treatment methods into a managementplan.
- Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.
- Work with patients in order to support compliance with andadherence to prescribed treatments. Provide information and advice onprescribed or over-the-counter medication on medication regimens, side-effectsand interactions.
- Prioritise health problems and intervene appropriately toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care.
- Support patients to adopt health promotion strategies thatpromote healthy lifestyles, and apply principles of self-care.
Communication
- Utilise and demonstrate sensitive communication styles, toensure patients are fully informed and consent to treatment.
- Communicate with and support patients who are receivingbad news.
- Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcomedifferent levels of understanding, cultural background and preferred ways ofcommunicating.

Qualifications
- Currently employed/self employed as advanced level nurse/advanced paramedic practitioner working in accordance with the four pillars of advanced practice. MSc or Masters in Advanced Practice. Relevant nursing/paramedic health degree or Digital Badge Portfolio. Independent prescribing qualification.
- Long term conditions qualification. Two examples of evidence of CPD related to Advanced Clinical Practice within previous three years. Mentor/teaching qualification.
