Job description

Responsibilities

• Conduct remote clinical assessments via telephone or video consultation
• Diagnose, manage, and where appropriate, prescribe for patients presenting with urgent or unscheduled healthcare needs.
• Make safe, evidence-based clinical decisions and escalates where necessary.
• Collaborate effectively with GPs and other clinicians to ensure safe and seamless patient care.
• Contribute to clinical governance, audit, and quality improvement initiatives.

• Access to a secure private workspace suitable for confidential consultations.
• A Windows 10 or Windows 11 laptop (MacOS not supported).
• Reliable broadband internet connection.

About the role

We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join a remote GP Out of Hours (OOH) team supporting patients registered in the Telford area. Qualifications
