Job description

The role will involveundertaking minor ailment clinics (primarily face-to-face appointments butincluding telephone triage if necessary), with or without prior triage bythemselves or another clinician. The post-holder will perform chronic conditionreviews, including QOF reviews. They will undertake home visits and care homereviews. It is desirable that thesuccessful candidate is a prescriber.

Main duties of the job

The clinician will assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated, undiagnosed problem. They willdiagnose and manage both acute and chronic conditions, with relevant clinicalsupport, and initiate and interpret and action tests/investigations asappropriate. The clinician will preferably be a prescriber or in the process ofgaining a prescribing qualification.

Responsibilities includereviewing and acting on documents/workflow as appropriate, working withpatients to support compliance with and adherence to prescribed treatments, andprioritising health problems, intervening appropriately to assist patients incomplex, urgent or emergency situations, including the initiation of effectiveemergency care.

The role also includessupporting patients to adopt health promotion strategies that promote healthylifestyles and applying principles of self-care, as well as recording accuratedata in patient records to an agreed standard.

The post-holder willcollaborate with other members of the Primary Health Care Team includingdoctors, nurses and other AHPs, accepting referrals and referring to them forspecialist care. They will be responsible for direct referral to and liaisonwith consultants and other professional agencies.

Job responsibilities

The post holders mainresponsibility will be to work within the clinical team to provide high qualitypatient care.

The post holder will provideclinical assessment and management for patients of all ages withundifferentiated and undiagnosed problems seeking a primary health careservice, including minor illness face to face consultations, home visits, carehome visits and telephone triage.

The post holder must haveexperience of assessing and treating common minor illnesses in a primary orurgent care setting.

The post holder may carry out homevisits in the locality for patients who are housebound and registered with thepractice as required.

The post holder may haveresponsibility for an agreed number of care homes for which they will beprimarily responsible for clinical care, including weekly care home rounds.

The post holder will work inpartnership with the care home lead GP, clinical team including practicepharmacists, community health and social care team members and care home staff.

Qualifications
  • Registered healthcare professional (NMC/HCPC/GPhC)
  • Masters level qualification in Advanced Clinical Practice or working towards it
  • Evidence of CPD
  • Current professional registration in good standing
  • Full MSc in Advanced Clinical Practice
  • Additional modules (e.g., minor illness, paediatrics, mental health)
  • PG Cert in Education, mentorship or supervision training
  • Leadership/management training
Experience
  • Significant post-registration clinical experience
  • Ability to assess, diagnose, and manage undifferentiated/undiagnosed conditions
  • Experience in acute care, minor illness, long-term condition management
  • Ability to interpret investigations (bloods, ECGs, imaging reports)
  • Recognising and managing deteriorating patients
  • Understanding of NHS primary care systems and structures
  • Experience specifically within a GP practice or PCN
  • Experience running own clinics (minor illness, LTC reviews)
  • Experience with home visits, frailty, or care home rounds
  • Experience in womens health or sexual health
  • Experience leading or contributing to QI projects
