Job description

The Advanced Clinical Practitioner (ACP) will have the ability to manage clinical care in partnership with individuals, families and carers, where appropriate. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes. ACPs are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed.

Responsibilities
  • Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, in line with core professional competencies of the ACP, including full participation in appraisal and supervision, and acting as role model for self‑development.
  • Provide autonomous clinical assessment and treatment, demonstrating clinical expertise at an advanced level, in physical health with elements of mental health, substance misuse and learning disabilities for patients within Health and Justice. Contribute to the delivery of programmes of quality improvement within Health and Justice settings.
  • Provide advanced and expert level of practice (including non‑medical prescribing) within Health and Justice, leading to improved patient outcomes.
  • Provide clinical leadership and management leading to the provision of safe, quality, effective care.
