A community health organization in Tees Valley seeks an Advanced Clinical Practitioner to manage clinical care and provide autonomous treatment within Health and Justice settings. The role offers responsibilities including advanced clinical assessments, leadership, and participation in quality improvement initiatives. The Advanced Clinical Practitioner (ACP) will have the ability to manage clinical care in partnership with individuals, families and carers, where appropriate. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes. ACPs are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed.
