Job description

Job Summary

We are looking for an individual who has present experience of working as an Advanced Clinical Practitioner within a cardiology setting. The successful candidate, with support from the senior medical and nursing team, will be expected to work autonomously, providing integrated care for our cardiology patients undergoing elective and urgent interventional cardiology procedures. You will also support admission avoidance pathways and assess patients in the Emergency department. You should have the experience and interpersonal skills required to bridge the gap between medical, nursing and AHP roles. You will be an advocate for Advanced Practice within the CSU and support continual improvements to ensure the safety of patients within the Cardiology service. You will demonstrate a high level of motivation and be committed to your own on-going professional development. You will be at the forefront of the evolution of the ACP role within our cardiology team. There will be opportunity to gain the skills and experience needed to evolve new patient pathways to enhance patient experience, and streamline the interventional service and support admission avoidance pathways. You will require evidence of clinical leadership, managing change and delivering teaching programs. You will have a flair for managing resources, people and situations effectively, whilst keeping a calm head in a challenging environment. This post is 10.5 hours per week.

Why you should join our Team
  • Work within one of the UK's leading largest and most advanced cardiology Departments.
  • Be part of a highly skilled and supportive multidisciplinary team.
  • Access excellent professional development and leadership opportunities.
  • Help shape the future of cardiology care across West Yorkshire and The Humber.
What you will bring to the role

Criteria:

  • Practice as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical procedures.
  • Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.
  • Initiate pharmacological intervention, using non-medical prescribing or patient group directives and administer medications as prescribed by others in accordance with the Leeds Teaching Hospitals policies.
  • Use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.
  • Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.
  • Develop practice appropriate to patient and service need through understanding the implications of new emergency research, technology, health inequalities and professional trends and developments.
  • Be involved proactively in developing strategies and undertaking activities that monitor and improve the quality of healthcare and the effectiveness of their own and others' practice.
  • Continually evaluate and audit own practice and that of others at individual and systems levels, selecting and applying valid and reliable approaches and methods which are appropriate to needs and context, and acting on the findings.
