Greater Manchester Mental Health NHS Foundation Trust is seeking an Advanced Clinical Practitioner to lead assessment, formulation and prescribing of care within the Inpatient Mental Health Unit. You will manage a caseload and drive discharge planning in line with protocols.
The role involves MDT collaboration, supporting clinical decision making, and contributing to the inpatient service pathway across Bolton and Greater Manchester.
The Bolton Mental Health Directorate has an exciting opportunity for a dynamic and enthusiastic individual to apply for this role, to improve the service delivery within the Inpatient Mental Health Unit. The successful candidate will play a central role in supporting the clinical and operational team to ensure high quality, recovery focussed care is delivered to our service users. The post holder will hold a caseload and will lead assessment, formulation and prescribing of care / treatment planning, including discharge planning within agreed protocols and procedures and be an integral part of the Multi Disciplinary Team supporting clinical decision making and supporting the service pathways. The post holder will also support with providing a seamless pathway within the inpatient services. The Advanced Clinical Practitioner will hold a case load and will lead assessment, formulation and prescribing of care / treatment planning, including discharge planning within agreed protocols and procedures. The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.
