Job description

Our teams are ambitious and committed to delivering excellent services to those within our care.Were recruiting Advanced Clinical Practitioners (ACP) and Advanced Nurse Practitioner (ANP) from a variety of professional backgrounds including nurses, pharmacists and paramedics if youre experienced, with a Masters level award or equivalent and keen to taken on an expanded role within Primary Care, we would welcome your application.

As an Advanced Clinical Practitioner, within our Urgent Treatment Centres, you will support existing clinical care to enhance the capacity and capability within our multi-professional teams.

You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat, refer or discharge patients. This role will be face to face supported by a variable rota covering weekdays, evenings, weekends and bank holidays.

  • Carry out consultations with patients taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis.
  • Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate,
  • Provide clinical advice and mentorship to junior clinical staff.

Job responsibilities

TheAdvanced Nurse Practitioner will be responsible for the assessment of healthand clinical needs of the patient. To undertake consultations with patients asan autonomous practitioner using advanced clinical practice skills, ownclinical judgement to diagnose, treat, refer or discharge patients

Key Responsibilities:

Undertake consultations with patients taking acomprehensive patient history, carry out physical examination use their expertknowledge and clinical judgement to identify the potential diagnosis

Exercise a higher degree of personal andprofessional autonomy using advances clinical practice skills to includeclinical interventions

Refer patients for investigations whennecessary/appropriate

Make a final diagnosis, decide on and carryout treatment, including the prescribing of medicine or referring to primary orsecondary care specialist services within local agreed pathways and protocolsas appropriate

Ensure that all treatments and care providedare based on best practice/evidence based care

Use their extensive to plan and provideskilled competent care to meet the patients health and social care needs,involving other members of the healthcare team as appropriate

Work with multi-disciplinary and multi-agencyteams in order to ensure patient needs are met

Asses and evaluate, with the patient, theeffectiveness of the treatment and care provided and make changes as needed

Use independent/supplementary prescribingskills for benefits of patient care

Work independently and as part of a team

Provide support/leadership/training/education

Assist in clinical audit and setting andmonitoring of service and unscheduled care

Identify and take appropriate actions toaddress risk to the patient, this will include:

Identification of emergency care needsrequiring despatch of a 999 ambulance

Safeguarding children and protection ofvulnerable adult issues which require referral on to other agencies in linewith local policies

Work independently and at the same timecontribute to the work of the whole team to ensure the delivery of aconsistently high level of service, this will include;

Taking responsibility and being accountablefor managing own workload, risk assessment and management, acting withinprofessional codes and appropriately to the Clinical Lead for advice andsupport

Undertaking other duties when required, suchas audits or administrative tasks

Identifying issues which may negatively impactupon service delivery and reporting these on to the Clinical Lead, OperationalTeam Leader/Shift Manager in a timely fashion

Acting as a clinical resource non-clinicalstaff who may require advice and support

Maintain contemporaneous records of theconsultation and action using the computer software and where necessary writtenrecords, in line with NMC guidance and local policies, guidelines andprocedures

Remain focused on the delivery of an excellentservice within an unpredictable, diverse and challenging workload

Communication

Work effectively as a team member, supportingand communicating with other professional colleagues and clinicians

Use critical thinking and effectivecommunication skills to holistically assess patients needs and determinerelevant culture, social, economic factors in a respectful and non-judgementalmanner

Experience
  • Minimum 5 year post registration experience
  • 2 years experience of working as a practitioner in a primary care environment, Emergency Department, Urgent Care Centre, MAU/EAU or other first contact setting, evidence of excellent assessment skills
  • Experience of having provided mentorship and/or coaching to other staff
  • Experience of having participated in service improvement activates including audit and research
Qualifications
  • Registered Nurse First Level Nurse or other healthcare professional qualification leading to registration
  • Evidence of degree level study or equivalent
  • Degree in advanced practice or equivalent (to include research an evidence to inform practice, clinical leadership, work-based learning, clinical history taking and examination skills, pathophysiology, investigations and immediate management, non-medical prescribing)
  • Evidence of completion of study in minor illness
  • Evidence of study in minor injury
  • Evidence of completion of study in paediatric minor illness
  • Evidence of study in paediatric minor injury
  • Masters level study in advanced practice
