A healthcare organization in Coalville seeks an Advanced Clinical (Nurse or Paramedic) Practitioner. The role involves comprehensive patient care, collaboration with a multidisciplinary team, and contributions to quality improvement. We have an exciting opportunity for an Advanced Clinical (Nurse or Paramedic) Practitioner to join theNorth West Leicestershire GP Federation to support the Primary Care Network, as part of our Nursing or Paramedic Team. The post holder will be supported and mentored by our Senior Advanced Nurse Practitioner or Senior Paramedic and Leadership Team, and work as part of a multi-disciplinary teamin a patient-facing environment. The post holder will be employed byNorth West Leicestershire Federation and be based in a practice within our PCN.
The post holder is an Advanced Clinical Practitioner (ACP)who, acting within their professional boundaries, will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of their care. They will demonstrate safe, clinicaldecision-making and expert care for patients. Experience of onlineconsultation, minor illness and women's health (HRT treatment, contraception, gynaecologicalcare, etc.) is essential for this role.
They will work collaborativelywith the multi-disciplinary team across all member PCN practices, to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding leadership as required, seekingquality improvement, and manage some aspects of the quality and outcomesframework to enhance services. In order to work at this level, NMC orHCPC requirements for advanced practice must be met.

The GP Federation operates the NorthWest Leicestershire Primary Care Network which is the largest within Leicester,Leicestershire and Rutland. The postholder will ensure that the practice integrates with community and localservice providers to help utilise skill mix, improve patient outcomes, ensurebetter access to healthcare, and help manage workload. The role is pivotal toimproving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within general practice.
The postholder will be required to identify any signposting, referrals, hospitaladmission or any other services identified within the care/healthcarepathway. Furthermore, you will adviseand provide support to members of the public/patients in leading healthylifestyles, disease prevention and health promotion.
Thepostholder will maintain, update, and monitor accurate systematic patientrecords of patient interactions, clinical care provided including adverseevents. You will also lead and ensure concise andaccurate PCN and Federation audits are conducted.
The post holder will be requiredto undertake any other duties as deemed reasonable within the PCN andFederation to support the adherence of the PCN DES Contract.
