This role provides a unique opportunity to contribute to a significant transformation programme, working directly with individuals, families, carers, providers and partner agencies to ensure support arrangements continue to meet people's needs and aspirations.

You will be joining a dedicated team focused on delivering one of the most important programmes currently underway across adults.

Why Join Us?

This is a fantastic opportunity to be part of a major programme that will help shape the future delivery of home care services across Cambridgeshire. Through your work, you will have a direct impact on improving outcomes for people with learning disabilities, physical disabilities and older people, while helping to implement a new and innovative Home Care Framework.

If you are looking for a role where you can make a real difference, develop your skills and be part of an ambitious and exciting programme, we would love to hear from you.

For more information or to chat about this opportunity please contact Laura Green on Laura.Green@cambridgeshire.gov.uk .

We are planning to hold interviews for these positions in the week commencing the 02 nd and the 09 th of November.

We are proud to serve the diverse communities of Cambridgeshire and want our workforce to be reflective of this diversity, which we believe benefits our employees, the organisation and our communities.

What will you be doing?

You will complete strengths-based reviews and work collaboratively with individuals, families, carers, providers and partner organisations to achieve positive, person-centred outcomes. You will refer individuals to advocacy services where appropriate and ensure care and support plans accurately reflect their current needs, outcomes and goals.
