What you will do
  • Conduct strengths-based assessments in line with the Care Act 2014 to identify and implement appropriate post-hospital discharge support.
  • Collaborate with NHS professionals, therapists, families, and community services to develop and coordinate effective discharge plans.
  • Complete Mental Capacity Assessments and assist in Best Interests Decisions when needed.
  • Identify and address safeguarding concerns to ensure the protection and well‑being of adults.
  • Provide information, guidance, and signposting to individuals and families about available community services and support options.
  • Apply eligibility criteria and explore funding options such as Continuing Healthcare (CHC) funding and local authority support.
  • Maintain accurate and up‑to‑date records, contributing to multi‑disciplinary team (MDT) meetings.
  • Promote reablement and independence, supporting individuals to regain autonomy with appropriate care.
