Job summary

Sanctuary Personnel seeks an experienced Adults Social Worker to support patients at James Paget University Hospital in Norfolk. The post involves inpatient assessments, care planning, and safe discharge coordination within a fast-paced multidisciplinary team.

The role offers flexible hours (part-time or full-time) with a competitive hourly rate of £35 per hour. A commitment to high-quality social care and safeguarding is essential.

Qualifications

• Qualified Social Worker with Social Work England registration.
• Experience in Adult Social Care settings.
• Strong knowledge of the Care Act 2014 and safeguarding principles.
• Experience with Mental Capacity Assessments and Best Interest decision-making.
• Familiarity with Continuing Healthcare reviews and processes.

Responsibilities

• Complete assessments with patients and families to identify needs and support requirements.
• Develop tailored care plans that promote independence and wellbeing.
• Coordinate safe and timely discharge arrangements with the multidisciplinary team.
• Provide ongoing support and review plans after discharge.
• Act as a key point of contact for healthcare professionals, service users and families to ensure coordinated care.
